Documentation includes intakes, assessments, treatment plans, referrals, etc., monthly reports, and correspondence. • Maintain confidentiality of consumer records & privileged information at all times. • Work with other agencies, groups and organizations to assure development of mutually consistent objectives compatible with existing goals, policies and practice. • Assists patients and client of health center in using effectively developed resource networks. • Participates in staff development, professional meetings, continuing education training conferences and shares information with other staff members. • Supports EEO and PI policies and established standards of Ben Archer Health Center. • Works in a culturally sensitive and safe manner. • Performs other duties as assigned.
